Types of Adult Assessments

  • When you’re concerned about behavior and emotions. Usually includes DSM-5-TR diagnoses. All potential diagnoses that could fit your experience will be considered, including but not limited to autism, ADHD, OCD, mood disorders (anxiety, depression), and trauma (PTSD, C-PTSD).

  • When you’re concerned about learning or memory. Usually includes cognitive (IQ) and academic testing. School and/or work accommodations will be recommended that align with Americans with Disabilities Act (ADA). Diagnoses and concerns considered can involve IQ (cognitive functioning), learning disabilities, ADHD, and executive functioning.

  • When you're concerned about how your potential learning disability or mental health diagnosis may impact your upcoming college, graduate school, or professional board exam (e.g., SAT, ACT, GRE, MCAT, LSAT, BAR, USMLE). Individuals with disabilities can apply for exam accommodations based on the needs demonstrated by our evaluation together.

What’s involved in an adult psych assessment?

  • Clinical Interviews (4-5 hrs)

    Dr. Stephanie meets “face-to-face” via telehealth video with you and any family/friends/partners as appropriate. Some of our conversation may feel informal, and some will be semi-structured asking about specific symptoms and experiences.

  • Standardized Questionnaires (self-paced)

    You and other important people in your life are sent standardized questionnaires via email to complete based on your assessment questions. This adds some quantitative (numbers) data to the information given in interviews. It also helps compare what you're experiencing to others your age and gender.

  • Performance Testing (if needed)

    This is usually only needed if you have concerns about learning, cognitive functioning (IQ), or academic skills. This is often required for high-stakes exam accommodation testing. We meet in person or via telehealth to complete cognitive (IQ) testing and/or academic achievement testing that provides more information on your learning and problem solving styles.

  • Visual Feedback Session + Report

    Dr. Stephanie meets with you to clearly explain the takeaways. The results of the assessment are summarized into a visual slide show presentation covering your strengths and needs, diagnoses, explaining important concepts, and recommendations that you can implement right away. This is saved as a PDF and sent to you along with a brief letter detailing your diagnoses.

    If you choose, a full narrative report with comprehensive details about each assessment result and areas of need is sent to you 4-6 weeks after feedback.

Fees

 

Virtual Assessment

$2,880 (6 hours of testing + brief summary)
$4,320 (6 hours of testing + written report)
$6,480 (6-8 hours of testing + IQ or achievement testing + written report)
$360 per additional testing hour

Each flat fee includes 6 hours of interviews, testing with the client, and the assessment feedback session. Any additional time with the client is billed at $360/hour. In-person testing requires $25 travel fee per visit. At the end of your assessment intake, you will be quoted the maximum amount expected based on your referral questions.

 

Consultation & Advocacy

$260 per hour + $25 travel fee per visit

It is usually helpful for me accompany you to school meetings or medical appointments to help explain the test results and help you advocate for needed services and supports. Adult clients often need support applying for accommodations with work or school. In-person advocacy beyond the clinic, when included with an assessment, is charged at $260 an hour plus a small travel fee to cover the cost of roundtrip mileage and time away from the clinic. An additional travel fee may be included if travel is required outside of 10 miles of Corvallis, OR.

In-Person Assessment

Oregon only

$360 per hour + $25 travel fee per visit

Sometimes in-person testing is required. If we are provided with a private, quiet space suitable for testing, I can bring assessment to your home, school, or workplace. When testing or observations must occur in person, I charge the standard $360 hourly rate plus a small travel fee to cover the cost of roundtrip mileage, time away from the clinic, and additional time required to coordinate. An additional travel fee may be included if travel is required outside of 10 miles of Corvallis, OR.

 

Insurance

Empowered Together Psychological Services is a private pay practice and does not accept insurance for payment. We provide superbills upon request that you may submit to your insurance company for out-of-network reimbursement. Please consult with your insurance provider for more information about your policy.
